  1. Determine if Your Child Qualifies
    • Children must have a medical diagnosis of Autism Spectrum Disorder or a related condition.
    • Families must have active insurance that covers ABA or EIDBI therapies. Medical Assistance, or MN Medicaid, covers EIDBI services for all eligible children. We are also in-network with BCBS MN and HealthPartners.  For questions about other insurance plans, please contact us. 
  2. Complete Intake Application
    • Please complete an “Intake Application”. You can email this back to us at contact@foundationsautismcenter.com or drop it off in the center. Once this is received, a member of the clinical team will review, reach out with questions, and determine initial eligibility for our program.  If it is determined that your child is eligible for services at Foundations, our team will discuss the timeline and next steps.   
  3. Join Our Waitlist
    • Our clinical team will communicate with you monthly to share any changes to our waitlist.  Once it appears there is a spot open for your child in one to three months, the clinical team will reach out and start the assessment process. The assessment includes a Comprehensive Multidisciplinary Evaluation (CMDE) as well as an Initial Assessment.  The CMDE and Initial Assessment will determine treatment goals, approximate start date for services, as well as the number of hours per week that are determined to be medically necessary for your child.
    • We offer tours of our center to families who have completed the “Intake Application” and are interested in joining our waitlist.

  • While we would be honored to support all children who need our services, the Early Intensive Developmental and Behavioral Intervention (EIDBI) Benefit is a Minnesota Health Care Program that requires a medical diagnosis of Autism Spectrum Disorder or a related condition. Joining our waitlist starts with the completion of our Intake Application. Once all steps have been completed, a member of our clinical team may reach out with questions to determine initial eligibility for our program. Once your application is complete, you may also schedule a tour with our team to see the center and meet our experts.

If qualifications and eligibility are met, your child will officially be on our waitlist. Once on our list, our clinical team will keep you up-to-date monthly to share your status. When a spot for your child will be open within one to three months, we will begin the assessment process. This process includes a Comprehensive Multidisciplinary Evaluation (CMDE), and an Initial Individualized Treatment Plan assessment. Both processes help our experts determine an individualized plan, approximate start date, and predicted hours of therapy per week. Then, the life-changing fun begins!
